Warangal: A person who claimed his name was Shivaiah filled his Abhaya Hastam application form with an image of Lord Shiva, at Mutharam village of Bhimadevarapally mandal in Hanamkonda district.

He mentioned his date of birth as January 1, 1200, and named his family as wife Parvati Devi and sons Kumaraswamy and Vinayakudu. The form did not have the mandatory house address or phone number.

The person applied for the five schemes — Maha Lakshmi aid for women, Rythu Barosa, Indiramma Indlu, Gruja Jyothi and Cheyutha — and submitted the form on the final day of the Praja Palana programme on January 6.

The Anganwadi teacher who was appointed to receive the forms gave him an acknowledgement receipt, and allotted the number C-1/205.
